Mrs WRATHALL had a medical history which included advanced dementia and she lived in a care home. On the 10th January 2026 she had an unwitnessed fall which resulted in a fractured neck of femur. Mrs WRATHALL underwent surgery to repair her femur on the 12th of January 2026 but suffered bone cement implantation syndrome, a known risk of the surgery and died. It is not possible to determine whether an accident or medical difficulty caused Mrs WRATHALL to fall. |
| 06/05/2026 | 06/05/2026 | ASHWORTH, Michael | On 23 November 2025 Michael ASHWORTH was found collapsed at 1 Mitre Street, Burnley having taken a mixture of street and prescription drugs including heroin and cocaine, plus alcohol, against a background of significant coronary atheromatous disease, and was taken to Royal Blackburn Hospital where, despite eventual return of spontaneous circulation after not less than 73 minutes, he continued to deteriorate and died on 25 November 2025. |

| 07/05/2026 | 07/05/2026 | ANGUS, Kay Lindsay | On 25 December 2025 Kay Lyndsey ANGUS was found collapsed at her home address of 63 Whitegate Close, Padiham having taken heroin and a number of other prescription and illicit drugs against a background of severe Chronic obstructive pulmonary disease, and despite best efforts she could not be resuscitated. |

| 07/05/2026 | 07/05/2026 | STONEHEWER, Robert PAUL | On 5 January 2026 Robert Paul STONEHEWER was found already deceased in the bedroom of his home address at 132 Lowerhouse Lane, Burnley. He had suffered an gastrointestinal bleed and aspiration of gastric content, whilst impaired in cognition and respiratory function as a consequence of taking a significantly higher than therapeutic level of pregabalin, together with codeine. |

Mr TAYLOR had a history of schizophrenia for which he received treatment for. His mental health had deteriorated prior to his death and for this reason it is not possible to determine his intentions at the time at which he secured the ligature. |

Mr FINLAY had a long history of chronic drugs misuse which contributed to the development of conditions which impaired his cardiac function. Mr FINLAY died as a consequence of coronary artery disease which was contributed to by emphysema and chronic polydrug use. |

Mr HUDSON's mental health had deteriorated in the months before his death which had lead to unusual behaviours. It is not possible to determine how Mr HUDSON came to be in the water or what his intentions were at that time. |

| 11/05/2026 | 11/05/2026 | WALKER, Geoffrey Maurice | Geoffrey Maurice WALKER died on the 16th December 2025 at the Royal Blackburn Hospital. Mr WALKER had attended the hospital on the 20th November 2025 to attend an outpatient appointment. As he entered the building via the revolving door it slowed or stopped which caused him to fall. Mr WALKER sustained traumatic fractures in the fall and was admitted for pain management. In the course of his admission, Mr WALKER developed infections and his pre-existing health conditions deteriorated. He died as a consequence of congestive heart failure and pneumonia but his death was contributed to by the injuries he sustained in the fall which impacted his ability to compensate for existing health problems. |

| 14/05/2026 | 14/05/2026 | SMITH, Colin Joseph | Colin Joseph SMITH died on 25 April 2026 at Flat 58 Pardoe Court, Junction Lane, Ormskirk in Lancashire. Mr SMITH collapsed following consumption of alcohol. Paramedics attended but Mr SMITH did not regain consciousness. He died as a result of a sudden cardiac event precipitated by alcohol related physiological stress |
