  • Support with mobility and moving safely around your home
  • Assistance with shopping, household tasks and getting out into your community
  • Companionship and regular visits to help you stay connected

For some people, more specialist support may also be available, including care from staff with additional training.

How care at home works

1

Talk to us

We will talk with you to understand your situation and what support you feel you need.

2

Assessment

An assessment may take place to look at what help would make the biggest difference to your day-to-day life.

3

Your support plan

We will work with you to agree a care and support plan that reflects your goals, preferences and what matters to you.

4

Ongoing review

Your support will be reviewed regularly to make sure it continues to meet your needs.

Your options

You may choose to arrange care yourself through a care provider, or you may be eligible for support through adult social care.

Advice and guidance are available to help you understand your options and make the right choice for your situation.
